Hidden Gems: Meet Rachel Siegel of NobleCare (Rachel Siegel NP, A Nursing Corporation)

Today we’d like to introduce you to Rachel Siegel.

Hi Rachel, please kick things off for us with an introduction to yourself and your story.
Hi, I’m Rachel Siegel. Born and raised in Milwaukee, Wisconsin, I completed my nursing education in the Midwest and began my career as an oncology nurse at Northwestern Medicine in Chicago. As I provided cancer care, I became deeply interested in balancing disease-focused treatment with care that prioritizes comfort and quality of life, leading me to practice in both oncology and hospice settings simultaneously. When I moved to Boston, I wanted to understand firsthand what happens when patients are transferred to the ICU from the oncology unit. This drove me to transition into critical care as a nurse in the adult medical intensive care unit, where I also served on the ethics committee.

That foundation paved the way for my move to Los Angeles with my husband. I spent the next several years as a critical care nurse at UCLA, during which I obtained my nurse practitioner license. Throughout my graduate studies, I maintained a dedicated focus on palliative care. I joined the Hospice and Palliative Nurses Association (HPNA)—the nation’s largest professional organization representing advanced practice nurses in the field—and served as president of the Los Angeles Regional Chapter (LARC).

Looking to blend acute and palliative care, I joined Landmark Health, an innovative medical group delivering comprehensive, in-home medical, behavioral, and palliative care to patients with multiple chronic conditions.
It was through my years at Landmark Health that I discovered a glaring gap in our healthcare system: the “Black Hole.” This is the vulnerable space between doctor visits and hospital stays. It’s where “small” questions pile up—like wondering whether a family member’s new symptom warrants a trip to the ER or urgent care, or if it’s safe to monitor. As anxiety creeps in, our fragmented healthcare system leaves families to figure it out.

That reality is what led to NobleCare—an advocacy and liaison service for aging adults specializing in Parkinson’s Disease and Dementia, offering both remote and in-person care. NobleCare gives patients and their families a way to ask questions, find peace of mind, an appointment accompaniment, reinforce and clarify instructions, review side effects and handle logistics between doctor appointments. As an on-call nurse practitioner advocacy service available via phone, text, and email, NobleCare is designed for those exact moments of uncertainty to be able to ask all sorts of questions. Today, I am a mother, an Adult-Gerontological Acute Care Nurse Practitioner, and the founder of NobleCare.

Can you talk to us a bit about the challenges and lessons you’ve learned along the way. Looking back would you say it’s been easy or smooth in retrospect?
I used to think ambition could only look one way—proving myself through constant intensity in prestigious hospital systems. I’ve since learned a deeper kind of ambition: one of alignment, where my goals and values serve my present life rather than a distant vision. Between celebrating eight years of marriage and raising my one-year-old and three-year-old, taking the professional leap to build NobleCare has meant recalibrating. I’m scaling NobleCare on a foundation of fierce advocacy for aging adults, while sustaining my capacity and staying rooted to my family.

I am learning that this shift doesn’t mean less success. In fact, it drives more efficient, effective, and impactful work. I am more engaged with my patients, present at home with my kids and husband, and valuing self-care more than ever before. While this has been the busiest season of my life, it has also been the most meaningful personally and professionally.

This mindset directly influences how I solve problems. When I ran into an inefficient intake protocol at a medical clinic, I didn’t try to force a flawed system to work. Pausing to find another path to support the patient completely shifted my perspective; I stepped back from the frustration and focused on finding a fix to get the patient what she needed. Ultimately, my greatest growth has come from realizing that friction isn’t a personal reflection—it is a signal to redirect my energy toward fixing the actual problem.
